Salary Range 5 £31,821 - £36,893 per annum
We are seeking a highly motivated and passionate Band 5 Nurse to join the multidisciplinary team on a temporary basis at the Community Inpatient Unit (CIU), which provides nursing and enablement services to patients within the Rehabilitation and Recovery Inpatient Unit.
CIU provide short term reablement for Services Users who may need to regain their confidence and skills on discharge from hospital or to get over a short-term health exacerbation. This includes assessing complex and specialist needs and developing, implementing and evaluating programmes of care, in order to support individuals to remain in community settings, in their own home, residential home or nursing home.
role will require you to undertake various clinical duties including
assessment, wound management, injections, and continence care as well as dealing
with acutely unwell. Ideally we are
looking for some with previous community experience however this is not
essential as you will received training and support.
In
return you will be part of a forward thinking MDT team, receive opportunities
for further development, regular supervision and support.

- Demonstrate evidence based practice

- Experience of working with multidisciplinary team
- Experience of delivering a range of clinical interventions
- Experience of assessing rapidly deteriorating patients
Desirable: - Research experience; - Mentorship/Teaching Experience
Qualifications
Essential: - Registered Nurse; - Recognised mentor qualification and/or Live on the mentors register OR willingness to work towards
Desirable: - Any post registration nursing qualification e.g. Community Formulary Prescribing, Tissue Viability

Essential
- Ability to maintain clear and accurate records
- Ability to communicate complex issues
- Ability to communicate information to clients/carers with sensory impairment, learning disabilities, dementia and challenging behaviour.
- Effective negotiation skills
- History of multi-disciplinary working and ability to liaise with other agencies
- Effective and accurate computer skills
- Ability to work on own initiative and as part of a team
- Ability to provide appropriate nursing care by prioritising and effectively managing an unpredictable workload
- Understanding of patient escalation
- Understanding of holistic needs in relation to both Health & Social Care needs, acute and long term conditions
- Ability to manage own workload
Desirable: - Supervisory experience; - Trained in administration of Intravenous medications; - Extended clinical skills
