Working as part of our

multi-professional specialist palliative care team, you will help patients

maintain independence, autonomy and quality of life through individualised

occupational therapy assessment, intervention and support. You will also play a

key role in discharge planning for patients returning home from the Inpatient

Unit, working closely with patients, families, carers and colleagues across

health, social care, community and voluntary services.

In this role, you will

support patients with activities of daily living, provide advice on symptom

management and equipment use, arrange adaptations and equipment where

appropriate, and undertake home assessments to support safe and timely

discharge. You will also contribute to rehabilitative models of care across

hospice settings.

You will act as a

specialist resource for colleagues, sharing up-to-date occupational therapy

knowledge and promoting evidence-based practice. The role also includes

contributing to education, training, service development, clinical governance

and the positive promotion of the Hospices values and services.

Heart of Kent Hospice

provides specialist care and support for adults living with terminal illness,

as well as their families, friends and carers. We help people live as fully and

as well as possible for the rest of their lives, with care that is compassionate,

expert and tailored to each individual.

You will join a supportive

Inpatient Unit team including nurses, healthcare assistants, clinical

administrators, doctors, therapy colleagues, housekeeping staff and volunteers,

working together with the wider Hospice multidisciplinary team to provide holistic

care and support.

Essential

  • Professionally Registered

Desirable

  • You will be professionally registered, able to demonstrate evidence-based practice, and comfortable communicating sensitively with patients, families, carers and professionals.
