Specialist Diabetic Nurse

The position is Part-Time 24-27hrs per week. The role is structured

with an organised work flow, with set appointment numbers and the Postholder

should expect a mix

of same-day and pre-booked diabetic review encounters.

Job

Title: Diabetic Specialist Nurse

Main

Purpose of Post: Triage and consultations. The role involves telephone triage, chronic disease management and medication monitoring. The focus of care

evolves around our diabetic patients.

Reporting

to: Advanced Nurse Practitioner Lead Nurse

Interface with: Patients

Teams

Location: Postholder will be required to work at The

Dussindale

Branch Surgery as required.

To act as

an autonomous practitioner who is able to work independently and in conjunction

with other members of the clinical team. The post holder will be an experienced

caring nurse who can demonstrate developed clinical and diabetic skills and be able to

demonstrate critical thinking whilst working within their professional

boundaries. They will work the Surgery clinical team, consisting of GPs,

ANPs, Practice Nurses and Health Care Assistants to deliver excellent primary

care services within the practice.

This role

will demonstrate safe, competent clinical decision-making and deliver expert

care, including assessment, diagnostic skills, treatment and will communicate

at all levels across boundaries.

The post

holder will work collaboratively with the multidisciplinary team to meet the

needs of patients, supporting the delivery of policy and procedures, and

providing nurse leadership as required.

A point

of contact within the practice for diabetic patients who require review and management of their chronic disease, that require either telephone or Face to Face

consultation.

Assessment, monitoring, diagnosis, treatment, prescribing, referral as appropriate and

deliver key elements of Surgery initiatives.

Instigate

invasive and non-invasive diagnostic tests or investigations, interpretation of findings and reports.

Directly

admits patients to secondary care outlets in medical need.

Refer

patients to other medical specialities for assessment where indicated.

To take

responsibility for making autonomous decisions in relation to patient care.

examine and assess patient needs from a physiological and psychological

perspective, and plan clinical care accordingly.

Assess,

diagnose, plan, implement and evaluate interventions/treatments for patients

with complex needs.

To

collect data and participate in regular Practice audit and research projects

Work

effectively with individuals in other agencies to meet patients and carers

needs

Recognise

and work within own competence and professional code of conduct as regulated by

the NMC

Produce

accurate, contemporaneous and complete records of patient consultation,

consistent with legislation, policies and procedures on the Surgery clinical

system

Deliver

care according to local and national guidelines and evidence-based care

Provide

expert Diabetes and nursing advice to others working within the team.

To maintain own 3 yearly registration with the

NMC

To maintain own continuous professional

development and ensure PDP kept up-to-date in line with the requirements of the

Work with the clinical and management team to

develop new services for patients

Contribute to practice incentive schemes, disease

management registers and Surgery targets set locally and nationally for patient

care.

Special Requirements of the Job

An understanding, acceptance and adherence to

the need for strict confidentiality.

Competence in the use of IT systems, i.e. coding

work as completed, with the ability to learn new software.

This job

Non medical prescribing qualification desirable but not essential.

Care Quality Commission

To ensure

or dangerous occurrences to the Patient Services Manager or Practice Manager.

Participate

and support Health & Safety statutory requirements and initiatives.

Maintain records of incidents and undertake risk assessments.

with the Surgery confidentiality and Information Governance Policies.

Take

annual appraisal and undertaking subsequent activity identified and agreed with

the line manager.
