We are offering an exciting opportunity for a Specialist Community Public Health Nurse (SCPHN) Health Visitor/School Nurse to represent the 0-19 Public Health Nursing Service within the Integrated Front Door. The Integrated Front Door in Barnsley consists of different agencies that come together and are co-located in the same office to work as the gateway into Childrens Social Care and Early Help services. The postholder will practice autonomously and as part of a wider multi-agency team, demonstrating and providing SCPHN expertise and knowledge to support effective information sharing with multi-agency partners, which facilitates continuity of care for children, young people, and families. You will also support the interpretation and sharing of health information that is necessary to inform multi-agency decision making in relation to children and young people in need of safeguarding in the Borough. Due to the complexities around the post and that it is within a co-located team, much of the work will take place within the office base at Wombwell Police Station.

Provide SCPHN expertise and knowledge to support effective information sharing with multi-agency

partners which facilitates continuity of care for children, young people, and families, within a co-located

designated office space. Support the interpreting and sharing of health information that is necessary to

inform multi-agency decision making in relation to children and young people in need of safeguarding in

the Borough.

As part of our team, you can be sure well do all we can to support you in being truly excellent. We offer a full induction programme, and excellent training packages to enable you to have the right skills to do your job, including regular supervision to help you recognise your continued professional development needs and support you to achieve your potential.

We offer working conditions that are flexible and agile that support good life work balance. By working together, with each other and with our communities, we can take pride in improving lives.
