We’re looking for a Social Worker to join our team. We are looking to appoint as a Level 2, Level 3 Social Worker or Senior Practitioner.

If you’re passionate about supporting young people to have the best possible start to adulthood, this could be the role for you.

The Specialist Adult Pathway Team (SAPT) supports young people and their families as they prepare for adulthood. We work closely with Children with Disabilities (CWD) and wider Children’s Social Care teams, alongside health, education and other partners, to make sure the transition into adult services is as smooth and well planned as possible.

You’ll be joining a team where you can build meaningful relationships with young people and families and make a real difference at an important point in their lives.

What you’ll do

As a Social Worker within SAPT, you’ll have the opportunity to:

  • Build positive relationships with young people and their families
  • Complete holistic, strengths-based assessments
  • Develop personalised care and support plans
  • Support young people to prepare for adulthood and greater independence
  • Work collaboratively with health, education, housing and community partners
  • Help ensure young people experience a coordinated transition into adult services

Every young person is different, so the work is varied, interesting and gives you the opportunity to use your professional judgement and creativity.

Who we’re looking for

You’ll ideally have experience of working with children or adults with disabilities, strong multi-disciplinary working skills and a good understanding of relevant legislation, including the Care Act, Mental Capacity Act, DoLS and SEND legislation.

Most importantly, we’re looking for people who are collaborative, committed and want to be part of a supportive team.

What you can expect from us

We know that good Social Work happens when practitioners feel supported.

You’ll join a friendly, welcoming and collaborative team, with approachable managers, regular advice surgeries and reflective supervision. We’ll support your continued learning and development, with opportunities to build your confidence, develop your practice and progress through our structured career pathway.

Our employees are at the heart of the services we provide, and we want to attract and retain the best people. In return, you’ll have access to a range of benefits, including:

  • Generous annual leave entitlement
  • High-quality training and career development
  • Excellent pension scheme
  • Family-friendly policies
