Job description

Temporary Social Worker (Temporary 12 months) Front Door & Prevention Team

We are seeking an experienced and motivated Social Worker to join our Front Door & Prevention Team on a temporary basis.

This is a fast-paced and rewarding role where you will work with adults at the first point of contact, supporting timely decision-making, risk management, and the delivery of strengths-based interventions that prevent, reduce, or delay the need for long-term care and support.

  • Manage referrals and undertake timely screening and triage of new contacts.
  • Complete Care Act assessments, Mental Capacity Assessments, and risk assessments.
  • Work collaboratively with individuals, families, health partners, and community organisations to identify preventative solutions.
  • Develop strengths-based support plans that maximise independence and wellbeing.
  • Provide information, advice, guidance, and signposting to appropriate services.
  • Manage safeguarding concerns in line with statutory duties and local procedures.
  • Maintain accurate records and complete assessments within required timescales.
  • Work effectively within a duty environment, prioritising urgent work and managing competing demands.

  • Qualified Social Worker with Social Work England registration.
  • Experience of working within Adult Social Care.
  • Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, safeguarding legislation, and relevant statutory guidance.
  • Experience of undertaking strengths-based assessments and risk management.
  • Strong communication, decision-making, and partnership-working skills.
  • Ability to work independently within a busy and dynamic service.

Desirable

  • Experience working in a Front Door, Prevention, Neighbourhood, or Community Social Work Team.
  • Knowledge of local health and social care pathways.
  • Experience of multidisciplinary working.

We are looking for someone who can hit the ground running, manage a busy workload, make proportionate decisions, and maintain a strong focus on prevention and person-centred practice. The successful candidate will be resilient, adaptable, and committed to achieving positive outcomes for adults and their families.

The postholder will be expected to undertake a certain amount of travelling in the course of his/her duties for which a casual car user allowance will be paid.

Hours may be required to be worked over 7 days, as per the needs of the service.
