We have a range of teams within Older People services, including locality based teams, both residential and community reviewing teams, crisis intervention and prevention of hospital admission teams, and sensory services. We can offer Hybrid working arrangements, flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, as well as promote your continuous Learning & Development.Older Peoples Team West - Prevention of Admissions TeamApsley, Hemel Hempstead37 hours per week | PermanentThe South West Prevention of Admission Team supports individuals who live in their own home or usual place of residence and are aged 65 or over, or under 65 with a confirmed dementia diagnosis, who are experiencing a deterioration in health or care circumstances that is likely to lead to a hospital, residential, or inpatient admission within 48 hours.The team covers South and West Hertfordshire, including the localities of St Albans, Dacorum, Watford, Hertsmere, and Three Rivers.About youEssential: • Social Work degree • Registration with Social Work England • Full Driving licence We welcome applications from candidates who currently reside in the UK with established proof of right to work documentation.
