Job description
Joining our Intermediate Care Team as a qualified and experienced Social Worker , you will play an integral role in supporting individuals with diverse and complex mental health needs. The team works closely with Mental Health Hospital Teams and Joint Commissioners to facilitate timely hospital discharge, identify and review eligible Section 117 aftercare needs, and promote independence through strengths-based interventions.
