Job description
Job summary We are seeking to recruit two Social Workers to work in our service. The post-holders will work with a defined caseload of patients, most of whom will be in their own homes, providing direct, holistic assessment and care, in liaison with colleagues. Social Workers within community mental health care for Older People are embedded within the Mental Health Teams which requires them to deliver the statutory duties under the Care Act 2014.
