The successful candidate will be responsible for providing one-to-one support, identifying issues that impact on patients' health and wellbeing, developing personalised care and supporting plans to address their needs, making onward referrals to community and voluntary sector services, and recording patient outcomes.

S&WSR is a 4 practice PCN supporting over 24,500 patients. The culture is encouraging, supportive, forward thinking and there is a commitment to continually improving the quality of healthcare focusing on its patients.

We are a regulated service under The Health and Social Care Act 2014 and therefore fall within the proposed regulation changes of 9 November 2021 which require staff who have face to face contact with patients to have completed a course of an approved COVID 19 vaccine, subject to limited exceptions, by no later than the 1st April 2022 .

Qualifications
Desirable
- Ideally have Diploma Level 2/3 in Health and Social Care, psychology, sociology or public health. or equivalent qualification or experience
Experience
Essential
- Experience of working within healthcare, the voluntary or community sectors, supporting vulnerable groups
- Strong listening and communication skills
- You will be familiar with local resources and services and how to access them
- Motivated by helping people, with care and empathy
- Ability to deliver person-centred support in a non-judgmental way
- Experience of completing holistic, person-centred care-planning assessments
- Understanding of the needs of small, volunteer-led community groups and the ability to support their development
- Commitment to collaborative working with all local agencies
- Experience and confidence in the delivery of lifestyle change interventions.
