The Social Care Practitioner role is within the Integrated care team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers and Occupational Therapists.

The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital.

Your role will involve

  • Completing Care Act Assessments with residents and their carers.
  • Holding and managing a case load.
  • Assisting the duty function of the team, and providing advice and signposting to residents.

1.To manage the allocated workload of the councils residents who may be either within their own homes, or other care settings including hospitals. Work in partnership with them, their families, and carers and with any other multidisciplinary team involved, in order to achieve positive outcomes that support independent living.

2.To undertake assessments and reviews in line with the Care Act 2014.

3.To facilitate as required the implementation of support interventions to meet the residents identified outcomes and achieve their optimum functioning and independence using the case management system.

4.To maximise the engagement of residents in the assessment and intervention process whilst ensuring appropriate safeguards against risk are in place.

5.To ensure all aspects of safeguarding vulnerable adults are considered and addressed with supervision and support in accordance with Borough policies and protocols.

6.To ensure that any identified need for equipment or minor adaptations is carried out under the appropriate guidance in accordance with current processes and policies.

7.To ensure that any handover of responsibility for subsequent assessments/specialists, assessments and support planning is seamless for the person.

8.To ensure that assessment and delivery of services is within the eligibility criteria and within budget constraints.

9.To maintain comprehensive and up to date case records that demonstrate good practice and evidence that the RBWM is discharging its duties in accordance with relevant legislation.

10.To provide appropriate advice, support, and advocacy to ensure that peoples best interests are maintained.

11.To manage and be accountable, with supervision and support, for own practice and to take responsibility for own professional development including mandatory training.

12.To attend meetings relating to the work of the team or work with the individual as required.

13.To work within the requirements of the relevant legislation, the national agenda, and the Policies and Procedures of the council as appropriate to the responsibilities of the post.

14.To contribute positively to the performance of the team, service, and unit, by being aware of, and contributing to achieving key performance targets. To assist in the collection of data that demonstrates levels of performance and supports good performance management.

15.To be part of the relevant teams duty function.

16.To attend multi-disciplinary meetings, case conferences, and best interest meetings, ensuring effective information sharing and decision-making
