We have a fantastic opportunity for a Social Care Practitioner to join our Integrated Mental Health Team in Adult Social Care at Wokingham Borough Council on a full-time, permanent basis. Opportunities to join this team don’t come up often, and you’ll become part of an experienced, stable and supportive team who know the service well and are passionate about the work they do.

As a Social Care Practitioner, you’ll work alongside experienced social care professionals, supporting adults with serious or suspected mental health conditions and their carers. Working under the supervision of a registered practitioner, you’ll get to know the people you support, understand what matters to them and help them find practical ways to stay safe, independent and connected to their community.

Adult Social Care helps people who need extra support to stay healthy, connected to their communities, and able to live fulfilling lives.

This role offers a clear development pathway, with the opportunity to progress to Grade 6 once you have met the required progression criteria.

You'll be joining an award-winning council and a service that was rated among the top-performing adult social care services in the country following our recent CQC inspection.

Wokingham Borough Council is committed to Social Care Future, a movement that aims to make sure everyone, regardless of age, health condition or disability, can live in a home they choose, maintain the relationships and activities that matter to them, and be part of a supportive community.

You’ll have access to an excellent benefits package, including:

  • 27 days annual leave, rising by five days after five years’ local government service, plus bank holidays
  • Local Government Pension Scheme
  • GP helpline, seven days a week
  • Employee Assistance Programme, including 24/7 confidential support
  • Free eye tests and flu vaccines
  • Free onsite gym (Shute End, Wokingham)
  • Salary sacrifice schemes – including car and cycle to work
  • Local and lifestyle discounts
  • Move through Menopause course
  • Sports and social clubs – running, football, cricket and more
  • Learning and development opportunities
  • Employee networks
  • And more!

No two days will be the same. You’ll manage your own cases, with support and guidance from experienced practitioners.

  • get to know people and understand what matters to them
  • complete assessments and support plans that help people stay safe and independent
  • give advice and help people find the right support
  • spot safeguarding concerns and work with others to keep people safe
  • work closely with families, health teams and other services
  • keep clear records and support registered practitioners with more complex cases.

Above all, you’ll listen to people, involve them in decisions and help them find support that works for them.

Candidate requirements

We’re looking for someone who is caring, organised and comfortable working with different people.

  • experience in health, social care or a customer-facing role
  • GCSE Maths and English, or equivalent
  • good communication and IT skills
  • an understanding of safeguarding and confidentiality
  • the ability to stay calm under pressure
  • a real commitment to helping people stay independent.
  • experience of supporting vulnerable adults or working in an Occupational Therapy setting.
