An exciting opportunity has arisen for an enthusiastic and motivated Band 6 Physiotherapist to join the newly established Burren Intermediate Care Bed...
- Provide specialist physiotherapy assessment, treatment and rehabilitation for patients with complex physical, medical and frailty-related conditions.
- Work as an autonomous practitioner within a supportive multidisciplinary team.
- Develop person-centred rehabilitation goals that maximise independence and recovery.
- Facilitate timely discharge planning and safe transitions back into the community.
- Support patients with a wide range of conditions including frailty, long-term conditions, neurological, medical and surgical presentations.
- Work collaboratively with nursing, occupational therapy, medical, social care and community colleagues.
- Contribute to service development, quality improvement initiatives and audit.
- Provide supervision and support to junior staff, support workers and students.
- Help establish and embed new clinical pathways and rehabilitation practices within the service.

Qualifications
Essential
- BSc (Hons) Physiotherapy or equivalent recognised qualification.
- Current HCPC registration as a Physiotherapist.
- Evidence of continuing professional development and commitment to lifelong learning.
Experience
Essential
- Experience assessing and managing patients with complex physical, medical and rehabilitation needs.
- Experience managing a clinical caseload and prioritising workload effectively.
- Experience working within a multidisciplinary team environment.
- Experience supervising students, support staff or junior colleagues.
Desirable
- Experience within community rehabilitation, intermediate care, discharge-to-assess or transfer of care pathways.
- Understanding of risk management, safeguarding and patient safety principles.
