We are currently looking for a dynamic and enthusiastic Senior Physiotherapist to join our expanding Intermediate Care Team. The vision of the service...
To undertake assessment of patients with complex and multiple pathologies using specialist clinical reasoning skills.
To identify patient needs, agree goals and provide appropriate therapy interventions, provide equipment and rehabilitation programmes.
To work across Pathway 1 (care at home) and Pathway 2 (care home beds) for the Intermediate Care service.
To work as an autonomous practitioner within the integrated multi-professional team.
To be responsible for patients on shared caseloads within community and/or care home settings.
To take a holistic approach to all therapeutic interventions, working across professional boundaries at times.

Education / Qualifications
Essential
- Degree or equivalent eg. BSc, MSc or Diploma entry level qualification into the Physiotherapy profession
- Current HCPC registration
- Documented evidence of own continuing professional development
Desirable: - Member of CSP; - Fieldwork/Practice Educator qualification
Experience
Essential: - Relevant post-graduate clinical experience in a variety of settings
Desirable: - Community experience; - Equipment knowledge and provision
Skills & Abilities
Essential
- Able to demonstrate commitment to high quality care and service provision
- Ability to work flexibly as part of a team
- Good communication skills
- Able to assess plan and implement care
- To support all members of the multi-disciplinary team
- Good documentation skills.
- Proficient IT skills on various platforms and programmes
Knowledge & Understanding
Essential: - Recognises the limits of own authority within the role; - Seeks and uses professional support appropriately; - Understands the principle of confidentiality

- Ability to travel independently around Cambridgeshire and Peterborough area meeting required timescales. A Full driving licence and access to a vehicle is essential.
