Are you a dynamic, experienced Occupational Therapist with a passion for providing excellent care to residents in care homes and enhanced assessment b...

To undertake a comprehensive assessment of patients within enhanced assessment beds and in care homes, including those with diverseorcomplex presentations;usingadvanced clinicalreasoning skills.

To formulate and deliver patient specific Occupational therapy programmes based on patients goals and needs. This will include groupwork sessions including breakfast groups and exercises classes etc.

To evaluate patient progress, reassess and alter treatmentprogrammes if required and to set patient centered goals.

To support the discharge process working as part of a multidisciplinary team to facilitate discharges from Harvest View and EAB beds. This will include completing post discharge assessments in the individual's own home.

To attend MDT meetings and feedback on patient plans and progress.

To provide effective daily guidance and supervision for junior staff including Support Workers.

To be a clinical educator and support students undertaking placements within the team.

Qualifications

Essential: - oDegree/Diploma in Occupational Therapy; - oRegistered with HCPC

Desirable: - oMember of specialist section with RCOT

Experience

Essential

  • Experience must include working within a community service / rehab / neurology or long-term conditions
  • Experience of working as part of an Occupational Therapy team and the wider MDT.
  • Experience of delegating to and supervising Occupational Therapy students and junior staff members.
  • Contribution to the clinical education of less experienced staff/ undergraduate Occupational Therapists.

Desirable: - oInvolvement in audit and research

Communication

Essential

  • Able to adhere to Trust and Departmental documentation standards. Able to write complex patient reports
  • Able to communicate effectively in face-to-face contact, telephone contact, e-mail contact and internal and external meetings.

Management experience

Essential: - oAble to organise and prioritise own caseload; - oAble to organise, supervise and delegate tasks to support workers
