In this role, you will champion independence and well-being, providing essential assessments under the Care Act 2014. You will collaborate with a diverse team of 32 professionals to deliver innovative care solutions for service users and their carers. Responsibilities:

Conduct comprehensive assessments and reviews of individual needs.Provide expert advice on equipment, adaptations, and therapeutic techniques.Link service users to vital universal and voluntary resources.Collaborate across inter-disciplinary teams to improve service delivery. Qualifications:
