Our Short Term Pathways Occupational Therapy Teams are fast-paced, creative and highly collaborative, supporting people to return home safely following a period of ill health, hospital admission or stay in a care setting.We are looking for an inspiring and motivated Senior Practitioner Occupational Therapist to join our West Kent Short Term Pathways Team. This is an exciting opportunity to lead and support a skilled team of Occupational Therapists and Occupational Therapy Assessment Officers, helping to shape excellent day-to-day clinical practice and drive positive team performance.In this rewarding role, you will work closely with Short Term Pathways Social Care colleagues, Health Discharge teams, Kent Enablement at Home, Community Health Services and Social Care Occupational Therapy Teams. Together, you will take a coordinated, preventative, and person-centred approach to maximising independence, reducing the need for long-term care and helping people achieve meaningful outcomes in their own homes.You will be supported to thrive from day one.
