East Coast Community Healthcare are currently looking to recruit an additional enthusiastic and motivated senior nurse to provide shared leadership to...

The out of hours team form part of the Primary Care Homes 24/7 urgent response, which aims to support people at home and avoid unwarranted admissions. The PCH works closely with GP's and other members of the multidisciplinary team 24/7 to provide an excellent standard of person centered nursing care to adult patients in their own homes and residential care, supporting patients to self manage whenever possible. The senior nurse plays a key role in supporting the transition of care within 24/7 PCH care and/or partner organisations, ensuring unplanned needs are managed out of hours and an appropriate follow up plan is in place. In addition to clinical work, the senior nurse will have supervisory and line management responsibility, supporting the development of a highly experienced and well-embedded out of hours team. The senior nurse is accountable for the effective delivery of high standards of evidence based nursing care, including through the use of audits and supervision. The service uses SystmOne as a clinical IT system to support mobile working.

Applicants will be required to participate in working weekend and bank holidays on a rotational basis.

A mobile phone, laptop and pool car are provided for use when on duty.

  • Develop excellent internal and external networks in order to promote Primary Care Network.
  • Contribute to the clinical governance agenda through participation in clinical risk assessment and management and clinical audit.
  • Critically evaluate research findings, national guidelines and implement changes in clinical practice as appropriate.
  • Create an environment conducive to effective working, respecting and supporting staff to deliver high quality clinical services.
  • Actively manage, change and foster innovation within proactive and reactive care.
  • To deputise for the Lead Nurse or Integrated Care Lead if required.

Work Together, Achieve Together

  • Work as part of the multidisciplinary Primary Care Home to deliver proactive and reactive services.
  • Work in collaboration with colleagues to develop care pathways for patients requiring a range of healthcare needs and reablement.
  • Work collaboratively with residential care homes and communitycolleagues to promote high standards of care.
  • Contribute to the review of current policies/ protocols/guidelines and the development of clinical pathways and outcomes for people requiring proactive and reactive services.
  • Work collaboratively with the Primary Care Home Team and the training department to assess the clinical training needs of staff.
  • To formulate an individualised management and/or treatment plan, using clinical reasoning skills and utilizing a wide range of treatment skills in order to formulate a specialised programme of care. This will ensure patients meet their re-ablement goals and promote independence.
  • To communicate effectively and work collaboratively with medical, nursing and therapy colleagues to ensure delivery of a coordinated multidisciplinary service. This will include discussion of patient care and case management.
  • To work collaboratively with colleagues in partner organisations towards a shared objective of seamless, patient-focussed service provision

  • Plan and organise a range of complex clinical activities in a wide range of community settings to ensure best practice is delivered across your designated area of responsibility.

  • To communicate assessment and treatment results to the appropriate disciplines in the form of reports or letters.
  • Take responsibility to ensure compliance with Health and Safety Policy, Fire and Environmental Waste Regulations.
  • Be responsible for understanding, following and implementing ECCH policies and procedures.
  • Manage individual performance, reporting areas of concern and non--compliance to the Integrated Care Lead.
  • Ensure staff compliance with ECCH risk management systems, proactively identifying risk, reporting and managing risk within area of responsibility.

The PCH out of hours service comprises of experienced registered nurses and health care assistants, working in teams of two. The teams work by geographical area, but also flex to support each other, prioritising patient need. Qualifications

Essential

  • *Professional Registration with Nursing and Midwifery Council (NMC) Registered Nurse Level 1 (Adult)
  • *BSc in Nursing Practice or equivalent
  • *Evidence of continued professional development and a professional portfolio.
  • *Leadership/ management qualification or willing to undertake
  • *Health Coaching programme or willing to undertake

Desirable

  • *Specialist Practitioner Qualification in District Nursing
  • *Community Nurse Prescriber v100 /v150
  • *Independent non- medical prescriber
  • *Mentorship / Teaching and Assessing Course (ENB 998) or equivalent

Experience

Essential

  • *Experience working within a similar field of nursing/healthcare
  • *Experience of working as part of a multi-disciplinary team
  • *Experience of working with frail older adults and awareness of the complex issues relating to this client group.

Desirable

  • *Previous experience at band 6 level.
  • *Clinical experience gained in a community/intermediate care setting.
  • *Student supervision/clinical education
