We are offering an exciting opportunity for a NMC Registered Band 6 Senior Nurse Co-ordinator to join our Discharge to Access (D2A), Home First Servic...

As a Senior Nurse Co-ordinator you will assess, manage, plan and deliver care whilst providing support to your peers and team leader.

As part of this role, you will also so be expected to:

Triage referrals for the discharge to assess pathway in line with the referral criteria.

Provide holistic complex assessment, planning, implementation and evaluation of care. For example, Chronic Disease Management, Tissue Viability, Palliative Care.

Supervise pre-registration students.

Work collaboratively and actively supporting and liaising with other health and social professionals and agencies.

Actasarolemodelandmotivateteam membersinworkingtowardsan innovativeandhigh standard of practice.

Participateinthedevelopmentofaneffectiveteamandthedevelopmentof productive working relationships throughout the Trust.

  • Take an active interest in working parties and groups within the Trust to develop and improve on service delivery, protocols and guidelines.

Qualifications

Essential: - RGN Qualification Degree; - Evidence of continuing professional development; - Physical Health Assessment Mentor/ Student Qualification

Desirable: - Certificate in management studies Nurse prescribing (V150/300)

Experience

Essential

  • Experience of working within a multi- disciplinary environment
  • Clinical experience in either an acute or community setting
  • Understanding of the aims and objectives of the national discharge to assess pathway and national reporting/ timescales.
  • Experience of making independent decisions
  • Experience of resolving complex issues/situations
  • Understand the importance of chronic disease management and rehabilitation with in the community

Skills, Knowledge and Experience

Essential

  • Working knowledge of professional practice across the Local Health Economy
  • Working knowledge of policies and procedures relating to health and safety and risk assessments
  • Assessment skills and evaluation of safe discharge
  • Proactive case management leading to timely discharges
  • Able to plan, pre-prioritise and delegating the patient caseload appropriately

Personal Attributes

Essential

  • Strong interpersonal and communication and leadership skills
  • Good organisation skills and ability to prioritise and work in a self-directed way
  • Able to drive
