The Band 6 Community Mental Health Nurse is a key member of the West Norfolk Community Mental Health Team (CMHT), providing specialist assessment, car...
- Deliver high-quality, recovery-focused mental health care in line with NSFT values, Trust policies and the NMC Code.
- Undertake assessments under appropriate supervision, contribute to care planning, deliver interventions and evaluate outcomes for service users.
- Monitor mental and physical health needs, including medication management, escalating concerns where appropriate.
- Develop and manage a caseload under supervision, working collaboratively with service users, carers and partner agencies.
- Undertake risk assessments and contribute to risk management and safeguarding processes.
- Maintain accurate clinical records and contribute to effective multidisciplinary team working.
- Participate in supervision, appraisal, mandatory training and continuous professional development activities.
- Contribute to quality improvement, audit and service development initiatives.

Qualifications
Essential
- Nursing degree or equivalent - Mental Health
- Practice Assessor and Practice Assessor training (NMC registrants) or Practice Educator (AHP & Social Worker Registrants)
- Ongoing professional development working towards degree level (if professional registration is following a diploma level course)
Desirable
- Recognised Leadership / Management training
- Higher degree or post graduate certificate in area of clinical / therapeutic practice
Experience
Essential
- Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
- Demonstrate effective risk assessment and risk management experience
- Experience in the delivery of clinical interventions
Skills
Essential
- Excellent communication and engagement skills
- Conflict resolution and Breakaway training (or requirement to undertake within first 12 weeks)
- Effective analytical and decision-making skills
Other
Essential
- Ability and willingness to travel independently to meet the needs and requirements of the service
Knowledge
Essential
- The ability to use clinical assessment tools and outcome measures effectively and collaboratively
- An understanding of the policies and guidelines both local and national
- Up to date knowledge of legal and ethical frameworks including Mental Health Act
