Your responsibilities will include
- Delivering clinical care and patient education in patients homes and virtually
- Supporting patients, families and carers throughout their treatment
- Working with NHS hospitals and multidisciplinary teams to support safe discharge and ongoing care
- Leading, coaching and supporting a small clinical team
- Monitoring clinical quality, patient outcomes and service KPIs
- Maintaining accurate and timely clinical documentation
- Managing and escalating clinical incidents and adverse events appropriately
- Supporting training, development and performance within the team
- Participating in the on-call service as required

Experience
Essential
- To be successful in this role, you will have:
- Current NMC registration
- Minimum 2 years post-registration experience
- Demonstrable leadership or mentoring experience
- Excellent communication and interpersonal skills
- A strong patient-focused approach
- Full UK driving licence and willingness to travel across your region
Desirable
- Desirable:
- Experience with CVAD, parenteral nutrition, or similar therapies
Qualifications
Essential
- NMC Registered Nurse with 2+ years post-registration experience
