In accordance with the current Trust objectives, the post holder will be responsible for ensuring that medicines reconciliations, on the assigned wards, are completed as soon as possible after admission. They will also be responsible for ensuring discharge prescriptions are dealt with quickly and efficiently, responding to medicines information queries and conducting audits
The clinical pharmacist will rotate through a number of specialities. These specialities will include medicine, stroke , trauma and orthopaedics, women and children, surgery, community and out patients.

Education and Qualifications
Essential
- Current registration with the GPhC as a pharmacist in the UK
- Evidence of Continuing Professional Development
- Master's degree in pharmacy.
Desirable
- Undertaking a post graduate qualification or similar clinical pharmacy training
Experience
Desirable
- Experience in clinical pharmacy service in NHS hospital
- Experience in audit
- Experience in delivering training.
Knowledge
Essential
- Good level of clinical knowledge
- Knowledge of Safeguarding and Equality and Diversity legislation
Desirable
- Knowledge of NICE
Skills and Abilities
Essential
- Demonstrates an enthusiastic approach to work.
- Demonstrates a good standard of verbal & written English
- Demonstrates accuracy and attention to detail.
