Job description

Nurse Care Navigator

Full-Time or Part-Time | Remote-First Role | Travel to Oxford 1 Day per Week

Are you an experienced healthcare professional with a background in private, concierge, or premium healthcare services? Do you thrive in a patient-centred environment where exceptional service is just as important as clinical expertise?

We're partnering with an innovative digital health provider that is transforming the way patients access and experience healthcare. This fast-growing organisation combines technology, clinical excellence, and a white-glove approach to deliver highly personalised care that puts patients at the heart of every decision.

This is a predominantly remote position, offering the flexibility to work from home while remaining connected to a collaborative clinical team. You'll attend regular team co-location sessions in Oxford approximately one day per week. The client is open to considering both full-time and part-time applicants, making this an excellent opportunity for experienced clinicians seeking greater flexibility.

As a Care Navigator, you'll be the primary point of contact for patients throughout their healthcare journey. Acting as the bridge between patients, clinicians, specialists and healthcare providers, you'll ensure every individual receives a seamless, professional and highly personalised experience.

This is a unique opportunity for someone who combines strong clinical knowledge with outstanding customer service skills and has experience delivering high-touch care within a private healthcare, concierge medicine, luxury health, or membership-based healthcare environment.

  • Be the first point of contact for patients, providing a warm, professional and reassuring experience.
  • Coordinate appointments, follow-ups and ongoing patient communications.
  • Triage patient queries and escalate clinical concerns appropriately.
  • Support patients through referrals, investigations and onward care pathways.
  • Research and connect patients with relevant healthcare services and providers.
  • Build trusted relationships with patients, ensuring they feel supported throughout their healthcare journey.

  • Support the day-to-day running of a growing digital healthcare service.
  • Utilise innovative AI-enabled healthcare technology platforms.
  • Identify opportunities to improve workflows, processes and patient experiences.
  • Resolve issues proactively, ensuring a seamless service for patients.
  • Work flexibly to meet patient needs when required.

  • Registered healthcare professional such as a Nurse Practitioner, Registered Nurse, Allied Health Professional or equivalent.
  • Proven experience within private healthcare, concierge medicine, executive health, luxury healthcare services, VIP patient services or similar premium healthcare settings.
  • Exceptional patient engagement and relationship-building skills.
  • Outstanding communication skills, both over the phone and in person.
  • Comfortable working in a technology-enabled environment.
  • Understanding of GDPR, patient confidentiality, safeguarding and CQC standards.
  • A proactive, adaptable mindset suited to a growing healthcare business.
  • Ability and willingness to travel to Oxford one day per week for team collaboration.
