An exciting opportunity has arisen for a Community Registered Nurse Associate to join our district nursing teams within the Chorley, South Ribble and ...

  • Provide safe, effective and evidence-based nursing care to patients in their own homes and community settings.
  • Undertake comprehensive assessments, develop care plans and regularly evaluate patients' needs.
  • Administer medications and treatments in accordance with clinical guidelines and individual care plans.
  • Provide wound care, continence care, catheter care and other clinical interventions as required.
  • Recognise and respond appropriately to changes in patients' health and wellbeing.
  • Support patients with long-term conditions, complex needs and end of life care.
  • Promote health, wellbeing, independence and self-management.
  • Maintain accurate and timely clinical records in accordance with professional and organisational standards.
  • Work effectively with GPs, community teams, hospitals, social care and other professionals to ensure coordinated care.
  • Provide support, advice and education to patients, families and carers.
  • Contribute to service development, clinical governance, audit and quality improvement initiatives.

Qualifications

Essential

  • Nursing Associate Foundation Degree
  • On-going registration with the NMC.
  • Evidence of Continued Professional Development that supports clinical practice.

Knowledge

Essential

  • Evidence of effective interpersonal skills
  • Ability to problem solve and make clinical decisions
  • Understanding of NMC standards and implications for practice

Skills

Essential

  • Excellent communication, listening, decision-making and problem solving skills.
  • Developed clinical skills with a clear ability to understand new concepts and ideas in clinical practice.
  • Can provide evidence of recent continued professional development within the scope of nursing.
