Medication Management

Caseload Management and Care Coordination

Individual and group work to support assessment and treatment

Medication management including handling controlled drugs

Managing a complex caseload

Individual and group interventions to support assessment and treatment

Collaborative care planning using Dialog +

Completing cognitive assessments

Physical health monitoring

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.

Qualifications

Essential

  • Dip/BSc (Hons) in Mental Health Nursing

Desirable

  • Phlebotomy

Qualifications

Essential

  • Currently Registered with appropriate professional body

Desirable

  • Evidence of further relevant post qualification training
  • Practice Assessor training

Experience

Essential

  • Post registration experience as a Mental Health nurse preferably in a mental health setting working with older adults with functional and organic mental health needs.

Desirable

  • Experience of working with Older Adults
  • Experience of carrying out assessments independently including risk assessment and mental capacity assessment
  • Experience of Care Coordination
  • Experience of carrying out individual work and group work

  • Ability to work independently and autonomously.

  • Developing care plans with individuals and carers

  • Experience of using Dialog + and co-producing care plans
