Supporting

Strategic Delivery

Support the delivery of the

Integrated Neighbourhood Team programme from a primary care perspective,

ensuring alignment between neighbourhood delivery models and the operational

realities of general practice.

Provide operational and

programme insight to support strategic decision making within programme

governance structures.

Support the Integrator

Delivery Board and programme leadership by ensuring that primary care

perspectives are reflected in programme implementation and delivery planning.

Establish clear operating model for INTs.

Defining roles, responsibilities and interfaces between organisations and

within Multi-disciplinary Teams (MDT) to ensure a coherent approach and remove

cross-system barriers.

Establish governance and escalation aligned to

the integrator PMO structure.

Review practice Memorandum

of Understanding (MoU) return and payment tracker and escalate any delay in

releasing payments.

Maintain senior oversight of

service model decisions, external requests, GSTT OD programme learning and

cohort spread readiness through the decision log and reporting rhythm.

Team

ways of working and INT identity established, ensuring shared understanding of MLTC, CYP and Frailty models across the

core team.

Southwark is implementing

Integrated Neighbourhood Teams (INTs) as a core element of its place-based

strategy to improve population health outcomes, tackle health inequalities and

deliver more coordinated, proactive and preventative care.

Integrated Neighbourhood

Teams bring together general practice, community health services, local

authority partners and voluntary sector organisations to deliver

neighbourhood-based services that respond more effectively to the needs of

local populations.

Primary care is fundamental

to the success of this model. General practice holds the most comprehensive

understanding of local populations and remains the principal access point to

the health system. Ensuring that neighbourhood delivery models are designed in

a way that is sustainable for primary care is therefore critical to the success

of the programme.

The Primary Care Operations

Director role has been established to support the operational integration of

primary care within Southwarks neighbourhood delivery structures and to ensure

that system transformation is grounded in the operational realities of general

practice.

The role will play an

important role in the Southwark INT Programme team, supporting the evolution of

the INT programme from mobilisation to mature delivery, helping to ensure that

neighbourhood models are operationally viable, integrated and capable of delivering

improved outcomes for Southwark residents.

Experience

Essential

  • Significant senior operational leadership experience within primary care or integrated care delivery
  • Experience supporting complex system transformation programmes
  • Proven ability to work across organisational boundaries within health and care systems
  • Strong understanding of the strategic and operational context of general practice and Primary Care Networks
  • Strong knowledge of Southwarks health and care system and partnership structures
  • Excellent partnership working and communication skills
  • Strong programme management and reporting experience

Desirable

  • Experience developing neighbourhood-based models of care
  • Experience working with PCN infrastructure
  • Experience working within Integrated Care Systems or place-based partnerships
