Practice Nurse

The successful candidate will provide a range of general practice nursing services including:

The successful candidate will hold current NMC registration and have experience working within General Practice or Primary Care.

Deliver a high standard of patient-centred nursing care.

Assess, plan, implement and evaluate programmes of care.

Undertake clinical assessment of patients presenting with identified healthcare needs.

Provide advice and support to patients and carers.

Refer patients appropriately to other healthcare professionals and services.

Work within professional competence and scope of practice.

Maintain accurate, contemporaneous and legally compliant records.

Ensure informed consent is obtained where appropriate.

Maintain patient confidentiality at all times.

Long-Term Condition Management

Participate in the assessment and management of patients with long-term conditions.

Promote self-management and healthy lifestyle choices.

Support achievement of QOF and other contractual requirements.

Monitor patients and escalate concerns appropriately

Cervical Screening and Women's Health

Undertake cervical screening in accordance with national guidance.

Promote participation in screening programmes.

Provide advice relating to women's health issues within competence and training.

Deliver childhood and adult immunisations and vaccinations.

Administer vaccines in accordance with national guidance, PGDs, PSDs and practice protocols.

Maintain vaccine cold chain requirements.

Monitor vaccine supplies and report any issues appropriately.

Encourage vaccine uptake amongst eligible patient groups.

Undertake wound management and dressings.

Remove sutures and clips.

Perform ear irrigation where appropriate.

Undertake venepuncture.

Record ECGs.

Perform spirometry where trained and competent ( if available )

Provide travel health advice and vaccinations where appropriately trained.

Carry out other treatment room procedures within competence.

Provide health education and health promotion advice.

Support smoking cessation initiatives.

Promote healthy lifestyle choices.

Support national and local screening programmes.

Encourage patients to engage in preventative healthcare.

Recognise and respond to safeguarding concerns involving children and adults.

Follow local safeguarding procedures and practice policies.

Report concerns appropriately and in a timely manner.

Maintain required safeguarding training.

Adhere to infection prevention and control policies and procedures.

Maintain safe standards of clinical practice.

Support infection control audits where required.

Ensure safe disposal of clinical waste and sharps.

Clinical Governance and Quality

Participate in clinical audit and quality improvement initiatives.

Contribute to incident reporting and significant event reviews.
