The successful candidate will work as part of our friendly and supportive practice team, providing safe, effective and patient-centred nursing care. Main duties will include:

  • Providing high-quality nursing care within your professional scope of practice.
  • Carrying out wound care and dressings.
  • Administering injections and vaccinations.
  • Taking blood samples and carrying out urine testing/dipstick analysis.
  • Recording routine clinical observations, including blood pressure, height and weight.
  • Childhood Immunisations
  • ECG's
  • Ear Syringing
  • Cervical Smears
  • Chronic Disease Clinics
  • Supporting the wider clinical team with patient assessments, treatment and ongoing care.
  • Working collaboratively as part of the multidisciplinary practice team.
  • Maintaining accurate and timely patient records and completing associated clinical administration.
  • Providing advice and support to patients where appropriate, promoting health and wellbeing.
  • Undertaking additional clinical duties as required by the practice.

Whilst some of the above is mandatory to apply, training and support will be provided to develop skills in areas such as childhood immunisations, spirometry, ECGs, ear syringing, cervical screening and chronic disease management clinics. This provides an excellent opportunity for the successful candidate to broaden their skills and develop within general practice.

The Practice Nurse will provide safe, effective and patient-centred care within their professional scope of practice, including:

  • Carrying out clinical assessments and developing appropriate care plans.
  • Providing travel health advice, including vaccinations, medication, safe sex, food and water hygiene and sun protection.
  • Delivering childhood and adult vaccinations in accordance with current national guidance and relevant Patient Group Directions (PGDs).
  • Supporting health promotion, including advice on healthy eating, physical activity, smoking cessation and alcohol awareness.
  • Undertaking ear irrigation and cervical screening in accordance with current guidance and professional standards.
  • Performing venepuncture and obtaining blood samples in line with practice procedures.
  • Assessing and managing wounds using evidence-based care.
  • Participating in chronic disease management clinics and supporting patients with long-term conditions.
  • Undertaking ECGs, suture removal, urine testing and other routine clinical procedures.
  • Collecting, handling, storing and dispatching clinical specimens appropriately.
  • Recognising and responding to anaphylaxis and other clinical emergencies in line with current UK guidance.
  • Performing CPR and responding appropriately to medical emergencies.
  • Supporting minor surgery and family planning services, including contraceptive procedures, where required and within competence.
  • Undertaking anticoagulation/Warfarin monitoring and providing appropriate patient assessment and advice.
  • Obtaining and accurately documenting informed consent.
  • Maintaining high standards of infection prevention and control.
  • Working collaboratively with the multidisciplinary team to provide coordinated patient care.

2. Administrative and Practice Responsibilities- Maintain accurate, contemporaneous patient records and ensure the clinical system is updated promptly.

  • Complete relevant documentation and coding to support safe patient care, monitoring and reporting.
  • Collect and maintain clinical data for audits, reports and performance monitoring.
  • Participate in practice, nursing and multidisciplinary team meetings.
  • Contribute to the development and review of practice policies, procedures and protocols.
  • Support seasonal vaccination programmes, public health initiatives and other practice campaigns.
  • Participate in practice projects and service development initiatives.

3. Leadership and Supporting Others- Take responsibility for your professional practice, learning and development, including clinical supervision.

  • Act as a positive role model and support colleagues to develop their skills.
  • Support and develop HCAs and other clinical team members within their scope of practice.
  • Mentor and support student nurses, medical students and other learners where appropriately qualified.
  • Promote a positive learning environment and share knowledge and good practice.
  • Contribute to service improvements and the development of clinical guidelines, protocols and standards.

4. Teamwork and Collaborative Working- Work effectively within the multidisciplinary team, understanding your role, responsibilities and professional boundaries.

  • Communicate clearly and delegate appropriately, considering competence and patient safety.
  • Prioritise and manage your workload effectively.
  • Maintain effective referral pathways to ensure patients receive appropriate care.
  • Work collaboratively with internal teams and external healthcare professionals.
  • Identify opportunities to improve patient care and contribute to service development.
  • Support local practice projects and initiatives.
  • Contribute to a positive, supportive and inclusive working environment.

5. Information and Technology- Use clinical systems and technology effectively to support patient care, planning, monitoring and communication.

  • Maintain accurate clinical coding and documentation to support reporting, recall, audit and clinical review.
  • Use appropriate clinical information resources to support patient care and maintain professional knowledge.
  • Handle patient information responsibly in accordance with GDPR, the Data Protection Act and practice policies.
  • Collect and present clinical information to support audit, service development and quality improvement.

6. Health, Safety and Infection Prevention- Work in accordance with practice health and safety, infection control, safeguarding and security policies.

  • Identify and manage risks and maintain a safe clinical environment.
  • Apply infection prevention and control measures in line with current guidance.
  • Maintain the principles of vaccine cold-chain management and ensure vaccines and medicines are stored, handled and disposed of safely.
  • Follow safeguarding procedures for children and vulnerable adults.
  • Follow procedures for fire safety, medical emergencies, equipment safety and security.
  • Understand workplace hazards, including COSHH requirements.
  • Complete mandatory and statutory training.
  • Support an inclusive approach to healthcare and recognise barriers to accessing services.

7. Emergencies and Challenging Situations- Respond promptly and appropriately to patients presenting with acute illness or clinical emergencies.

  • Recognise when additional clinical support or emergency services are required and escalate appropriately.
  • Respond professionally to challenging, distressed or aggressive behaviour in accordance with the practice's Zero Tolerance Policy.
  • Maintain personal and patient safety in potentially high-risk situations.

8. Communication and Patient Support- Communicate clearly, professionally and compassionately with patients, carers, families and colleagues.

  • Ensure patients understand their assessment, treatment and planned care, supporting informed decision-making and consent.
  • Adapt communication to individual needs, including language, culture, health literacy, disability and preferred communication methods.
  • Provide appropriate support when discussing sensitive or difficult information.
  • Advocate for patients where appropriate and ensure their views and preferences are considered.
  • Communicate effectively with external healthcare professionals involved in patient care.

9. Service Development and Improvement- Work in accordance with practice policies, professional standards, clinical guidance and national recommendations.

  • Contribute to the development of clinical services and participate in audit and quality improvement.
  • Use feedback, data and learning to identify opportunities to improve patient care and experience.
  • Support the implementation of new initiatives and changes to practice.

10. Clinical Governance and Quality- Work within your professional scope of practice and the NMC Code, taking responsibility for clinical decisions and actions.

  • Maintain accurate, contemporaneous and complete clinical records.
  • Deliver evidence-based care in line with national guidance and agreed practice protocols.
  • Use supervision, peer review and feedback to maintain and improve standards.
  • Participate in clinical audit and quality improvement programmes.
  • Identify and escalate clinical risks, incidents and near misses appropriately.
  • Contribute to complaints, incident and significant event reviews, identifying learning and improvements.
  • Safeguard children, young people and adults at risk and follow relevant legislation and guidance.
  • Recognise concerns relating to domestic abuse, substance misuse and vulnerability and refer appropriately.
  • Use practice resources responsibly and understand the financial implications of clinical decisions.
  • Contribute to a culture of openness, learning and continuous improvement.
  • Raise concerns appropriately, including through whistleblowing procedures where necessary.
  • Manage time, workload and resources effectively.

11. Learning, Development and Education- Take responsibility for maintaining and developing clinical knowledge and professional skills.

  • Maintain an up-to-date CPD record and participate in supervision, appraisal and development activities.
  • Complete mandatory training and undertake additional training relevant to the role.
  • Work with the Practice Management Team to identify training and development needs.
  • Share knowledge and learning with colleagues to promote good practice.
  • Support students and colleagues undertaking clinical training where appropriately qualified.
  • Provide education and health promotion advice to patients, carers and families.
  • Make effective use of learning opportunities and share relevant learning with the wider team.

12. General Responsibilities- Maintain professional standards and act in accordance with the NMC Code and relevant legislation.

  • Maintain confidentiality and respect patient privacy.

  • Work flexibly as part of the Practice Nursing Team and wider multidisciplinary team.

Ty Bryn Surgery is a well-established and forward-thinking NHS GP practice based in Trethomas, Caerphilly. We provide high-quality, patient-centred care to our local community and are proud to be part of the wider NHS Wales primary care service.

We are a friendly and supportive practice with a multidisciplinary team of approximately 35 staff, including GPs, nurses, healthcare professionals, administrative and reception colleagues, supported by an experienced management team. We cross-cover three different sites; Trethomas, Machen & Llanbradach.
