• Long-term condition management, including asthma, COPD, diabetes, hypertension, and cardiovascular disease.
• Cervical cytology screening.
• Childhood, adult, seasonal and travel immunisations and vaccinations.
• Wound care, dressings, and removal of sutures/clips where appropriate.
• Venepuncture and specimen collection.
• NHS Health Checks and health promotion activities.
• ECG recording and other clinical procedures.
• Lifestyle advice and support relating to smoking cessation, weight management, physical activity, and general wellbeing.
• Supporting patients with preventive healthcare and screening programmes.
• Maintaining accurate, contemporaneous patient records using the practice clinical system.
• Ensuring clinical areas and equipment are maintained in accordance with infection prevention and control standards.
• Participating in clinical audits, quality improvement activities, and practice development initiatives.
• Working within professional competency and seeking support where appropriate.

• Assess, plan, implement, and evaluate patient care.
• Recognise and respond appropriately to safeguarding concerns.
• Support the management of patients with chronic diseases through monitoring, education, and treatment reviews.
• Identify and report significant events, incidents, or risks in accordance with practice procedures.
• Contribute to the achievement of Quality and Outcomes Framework (QOF) targets and enhanced services where appropriate.

• Develop and maintain positive working relationships with colleagues and external healthcare professionals.
• Communicate effectively and sensitively with patients, carers, and stakeholders.
• Work collaboratively within the multidisciplinary team to ensure coordinated patient care.
• Participate in team meetings and training sessions.

• Maintain NMC registration and professional revalidation requirements.
• Participate in annual appraisal and personal development planning.
• Undertake mandatory and role-related training.
• Keep up to date with current clinical practice, legislation, and national guidance relevant to primary care nursing.

Desirable

• Experience of providing nursing care within primary care, community, or similar healthcare settings.
• Knowledge of chronic disease management.
• Experience of immunisations and vaccinations.
• Knowledge of infection prevention and control requirements.
