Care Navigator (Primary Care Nurse / Nurse Practitioner)
Full-time | Hybrid (Remote + In-person)
Oxford area (or able to travel regularly)
Modern healthcare can feel fragmented — we’re building something better.
We’re an clinician-led digital health company delivering highly personalised, relationship-based care. Our model combines clinical excellence, technology, and a premium patient experience — bringing continuity, coordination, and compassion back into healthcare.
We’re looking for an experienced Primary Care Nurse or Nurse Practitioner to join us as a Care Navigator — a pivotal role at the heart of our service.
This is not a traditional nursing role.
As a Care Navigator, you’ll be the first point of contact for patients, combining clinical expertise with triage, coordination, and relationship-building. You’ll guide patients through their healthcare journey, ensuring they feel supported and understood at every step.
You’ll work closely with GPs and senior clinicians, while helping shape how a modern, digitally enabled practice operates.
- Act as primary contact, delivering a personalised patient experience
- Triage queries, assess risk, and escalate appropriately
- Coordinate appointments, referrals, tests, and follow-ups
- Support patients navigating NHS and private pathways
- Provide continuity, reassurance, and advocacy
Clinical Coordination
- Work alongside clinicians to deliver safe, joined-up care
- Escalate clinical concerns appropriately
- Maintain accurate records in line with CQC and GDPR
Operational Development
- Help design care pathways and service models
- Engage with and improve digital/AI-enabled systems
- Support workflow development in a growing startup
- Registered Nurse or Nurse Practitioner (or equivalent AHP)
- 5–10 years’ experience, ideally in primary care
- Strong triage and clinical assessment skills
- Patient-centred, empathetic approach
- Excellent communication and relationship-building ability
- Experience in NHS primary care and/or private settings
- Understanding of CQC, confidentiality, and GDPR
- Comfortable using digital tools
- Able to balance clinical and administrative work
Personal Attributes
- Warm, professional, and personable
- Proactive and highly organised
- Adaptable and resilient
- Motivated to improve patient experience and outcomes
- Be part of a new model of primary care focused on quality and continuity
- Work alongside experienced GPs and clinicians
- Shape how care is delivered in a growing organisation
- Flexible hybrid working
- Supportive, high-trust culture
- Competitive salary with potential equity opportunity
