Practice Nurse - Park View Health Partnership

role focuses on the delivery of high-quality, evidence-based nursing care

to patients with chronic conditions, alongside the provision of preventative

healthcare to the wider practice population. You will use your

clinical expertise and professional judgement to assess, plan, implement, and

evaluate patient care, demonstrating critical thinking, accountability, and

excellence in every interaction. Working innovatively and

flexibly, you will contribute to service improvement and support the effective

delivery of care across the practice. The role requires commitment,

adaptability, and the ability to work resourcefully as part of a
