- Deliver diabetes reviews and ongoing management.
- Provide patient education and lifestyle advice.
- Monitor and optimise diabetes treatment in line with national guidelines.
- Undertake chronic disease management clinics.
- Administer vaccines and immunisations.
- Perform cervical screening, wound care and health promotion activities as appropriate.
- Maintain accurate clinical records and contribute to quality improvement initiatives.
We are looking for two experienced, compassionate and motivated Practice Nurses with Diplomas in Diabetes, to join our friendly primary care team. The successful candidates will play a key role in delivering high-quality nursing care with a particular focus on diabetes management. There are two positions available: 1x 30 hours per week and 1 x 20 hours per week.

Qualifications
Essential
- Registered Nurse with current NMC registration.
- Diploma in Diabetes Care.
- Experience working in primary care.
- Competent in managing patients with diabetes, including initiating
- and reviewing treatment plans.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary
- team.
Desirable
- Experience in managing other long term conditions (e.g. Asthma
- COPD).
- Cervical cytology qualification.
- Immunisation and vaccination training.
- Travel health and wound care experience.
- Knowledge of SystmOne.
