Clinical duties will include

  • Wound care and dressing changes
  • Routine and travel vaccinations
  • Long Term Condition (LTC) reviews (e.g. asthma, COPD, diabetes, hypertension)
  • Infection control audits and action planning
  • Patient Group Directions (PGDs) compliance and use
  • Vaccine stock management and cold chain maintenance
  • Travel vaccination forms processing and stock control
  • Data logger monitoring
  • HBPM (home blood pressure monitoring) review, scanning, and follow-up actions
  • Vaccine fridge temperature checks and escalation
  • Legionella checks
  • Clinical room stock management and replenishment
  • Receipt, organisation, and storage of deliveries
  • Filing and processing of pathology results
  • Sample handling and processing
  • Emergency drugs and equipment checks
  • General practice nursing tasks as required

Essential requirements

  • Registered Nurse (NMC registered)
  • Significant experience in general practice/primary care nursing
  • Competence in cervical screening (smears)
  • Experience in vaccination programmes and cold chain management
  • Ability to manage LTC reviews independently
  • Strong understanding of infection prevention and practice governance
  • Excellent organisational and communication skills
  • Ability to work autonomously while contributing to a multidisciplinary team

Jacksdale Medical Centre is seeking an experienced and highly competent Practice Nurse to join our friendly and supportive team.

We are looking for a confident, proactive clinician with significant primary care experience who is able to work autonomously, manage clinical responsibilities effectively and contribute positively to our multidisciplinary team.

The successful candidate will provide a range of general practice nursing services, support our vaccination programmes and contribute to essential clinical governance and practice management activities.

Key ResponsibilitiesClinical responsibilities will include:

  • Wound care and dressing changes
  • Routine and travel vaccinations
  • Long-Term Condition (LTC) reviews, including asthma, COPD, diabetes and hypertension
  • Cervical screening
  • HBPM (home blood pressure monitoring) review, scanning and appropriate follow-up
  • Processing and handling clinical samples
  • Filing and processing pathology results
  • General practice nursing duties as required

Clinical Governance & Practice ResponsibilitiesThe successful candidate will also contribute to:

  • Infection prevention and control audits and action planning
  • Patient Group Directions (PGDs) compliance and appropriate use
  • Vaccine stock management and cold-chain maintenance
  • Vaccine fridge temperature monitoring and escalation of concerns
  • Data logger monitoring
  • Travel vaccination forms and stock control

  • Supporting the practice with quality improvement and governance activities

Essential Requirements- Registered Nurse with current NMC registration

  • Competence in cervical screening
  • Experience with vaccination programmes and cold-chain management
  • Ability to independently undertake and manage Long-Term Condition reviews
  • Good understanding of infection prevention and control and clinical governance
  • Excellent organisational and communication skills
  • Ability to work autonomously while working effectively as part of a multidisciplinary team

Desirable Skills- Cervical screening

  • Spirometry
  • FeNO testing

Working Pattern2125 hours per week across Monday, Thursday and Friday.

  • Monday: 3:00pm8:00pm
  • Thursday and Friday: Hours between 8:00am and 6:30pm, to be agreed

A regular weekly commitment across these three days is required, with exact hours discussed and agreed at interview.

Experience

Essential

  • Current NMC registration as a Registered Nurse
  • Experience working as a Practice Nurse within general practice/primary care
  • Experience undertaking Long-Term Condition reviews, including asthma, COPD, diabetes and hypertension
  • Competence and experience in cervical screening
  • Experience administering routine and travel vaccinations
  • Experience of vaccine cold-chain management, including fridge temperature monitoring and stock management
  • Knowledge and understanding of infection prevention and control within a primary care setting
  • Understanding and experience of working with Patient Group Directions (PGDs)
  • Ability to work autonomously and independently manage clinical responsibilities
  • Good clinical assessment, decision-making and follow-up skills
  • Excellent organisational and time-management skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively as part of a multidisciplinary team
  • Commitment to providing safe, effective and patient-centred care
  • Ability to maintain accurate and confidential clinical records
  • Willingness to undertake practice governance, audit and quality improvement activities

Desirable

  • Experience undertaking spirometry
  • Experience undertaking FeNO testing
  • Additional training or qualifications relevant to general practice nursing
  • Experience in travel health and travel vaccination
  • Experience contributing to clinical audits and quality improvement projects
  • Experience with infection prevention and control audits
  • Experience managing practice vaccine stock and cold-chain processes
  • Experience with clinical governance within a general practice setting
  • Experience using SystmOne or another GP clinical system
  • Experience working within a small, multidisciplinary primary care team
  • Evidence of continuing professional development relevant to primary care nursing

Qualifications

Essential

  • Registered Nurse with current NMC registration
  • Relevant nursing qualification (Level 1 Registered Nurse)
  • Evidence of relevant continuing professional development (CPD)
  • Recognised training/competence in cervical screening
  • Recognised training/competence in immunisation and vaccination
  • Training and competence in vaccine cold-chain management
  • Training and understanding of infection prevention and control
  • Knowledge and understanding of Patient Group Directions (PGDs)
  • Evidence of maintaining clinical competencies relevant to general practice nursing
