We

are seeking a confident, clinically adept Registered Nurse to join our

team. In this role, you will focus on

managing and supporting patients living with chronic and long-term conditions

(such as diabetes, respiratory diseases and cardiovascular conditions). You will work autonomously, acting as a

crucial bridge between patients, GPs, clinicians and the wider

multidisciplinary team to improve patient outcomes, prevent hospital admissions

and promote proactive health.

Glovers Lane is a forward thinking 4 partner practice at the heart of the Netherton community, caring for over 7800 patients. As a proud teaching practice we currently support four GP registrars and regularly welcome medical students throughout the year creating a lively and supportive learning environment.

Our clinical team is made up of approachable and dedicated colleagues including a clinical pharmacist, an Advanced Nurse Practitioner, a Practice Nurse, A Salaried GP and a Health Care Assistant. They're supported by our Practice Manager, Deputy Practice Manager, on administrator and a team of eight part-time receptionist who help keep the practice running smoothly. Together we work collaboratively to provide compassionate, patient centred care.

Qualifications

Essential

  • Registration: Active NMC Nursing and Midwifery Council PIN with no restrictions.
  • Experience: Post-registration experience with a solid background specifically managing chronic diseases in a UK primary care or community setting.
  • Clinical Skills: Advanced competence in running chronic disease clinics and managing complex long-term conditions.
  • Qualifications Desirable: Post graduate qualifications in specific long-term conditions e.g., Asthma/COPD diploma, Diabetes modules or an independent prescribing qualification V300.
  • Personal Attributes: Strong communication, empathy, and patient-engagement skills with the ability to work both autonomously and as part of a team.
