Practice Nurse - Chronic Disease Management and Minor Illness

Plan and deliver structured

reviews for patients with long-term conditions, including diabetes, asthma,

COPD, hypertension and cardiovascular disease.

Undertake diabetes checks and

nurse-led reviews, including relevant history, lifestyle assessment, foot

screening, blood pressure and weight monitoring, review of blood and urine

results, medicines adherence, care planning and appropriate referral or escalation.

Assess patients presenting with

minor illness, provide evidence-based advice and treatment within competence

and agreed pathways, identify red flags and arrange timely escalation or

referral.

Assess patients holistically,

agree personalised care plans and support self-management.

-

Contribute to QOF, prevention,

screening, recall, audit and quality-improvement activity.

Undertake agreed core practice

nursing activity according to competence and service need, which may include

immunisations, injections, wound care, cervical screening, ECGs and spirometry.

Maintain accurate electronic

patient records and work collaboratively with the wider primary care team.

Job purpose

To provide high-quality general practice

nursing care with a primary focus on chronic disease management, including

diabetes checks and nurse-led diabetes reviews, and the assessment and

management of minor illness. The post-holder will work autonomously within

professional competence, practice policies and agreed clinical pathways.

Diabetes care and checks

-

Undertake structured nurse-led

diabetes reviews and interim checks in accordance with current guidance,

practice protocols and individual competence.

-

Take a relevant clinical

history and assess symptoms, lifestyle, self-management, medicines adherence

and risk factors.

-

Undertake or arrange

appropriate monitoring, which may include blood pressure, weight/BMI, foot

screening, urinalysis and review of blood and urine results.

-

Review progress against

personalised targets, provide education and lifestyle advice, agree care plans

and support self-management.

-

Recognise abnormal findings,

deterioration, complications and safeguarding concerns, and refer or escalate

appropriately.

-

Document care accurately, code

activity correctly and contribute to recall, QOF, audit and quality

improvement.

Minor illness

-

Assess patients presenting with

minor illness using appropriate consultation, examination and

clinical-reasoning skills.

-

Provide evidence-based advice,

treatment and safety-netting within competence and agreed practice pathways.

-

Recognise red-flag symptoms,

diagnostic uncertainty and deterioration, and seek urgent clinical support or

refer appropriately.

-

Use patient group directions

and prescribe only where appropriately qualified and authorised.

Maintain clear records of

assessment, advice, treatment, safety-netting and follow-up.

Wider chronic disease management

-

Undertake structured reviews

and ongoing monitoring for long-term conditions, including asthma, COPD,

hypertension and cardiovascular disease.

Agree personalised management

plans, promote shared decision-making and support self-management.

Review treatment adherence and

effectiveness, interpret relevant results and escalate changes outside

competence.

Core nursing and prevention

Provide health promotion and

lifestyle advice, including smoking cessation, diet, exercise and

cardiovascular risk reduction.

Undertake agreed duties

according to competence, which may include immunisations, injections, wound

care, cervical screening, venepuncture, ECGs, spirometry and physiological

observations.

Clinical governance and teamwork

Practise in accordance with the

NMC Code, legislation, evidence-based guidance, practice policy and

professional scope.

Maintain accurate, complete and

contemporaneous electronic records.

Comply with safeguarding,

infection prevention, confidentiality, information governance, medicines

management, health and safety and cold-chain requirements.

Work collaboratively with the

practice team, PCN and other health and care professionals.

Contribute to meetings, audit,

service review, protocol development and quality improvement.

Maintain NMC registration,

revalidation and a current professional portfolio.
