The Practice Nurse will work as part of the multidisciplinary practice team to provide safe, effective and patient-centred nursing care. Main duties will include:

Providing general nursing care to patients.
-
Undertaking routine and chronic disease reviews, including areas such as diabetes, asthma, COPD, hypertension and cardiovascular disease where appropriately trained.
- Providing immunisations and vaccinations, including childhood and adult programmes.

Undertaking cervical screening.
-
Providing wound care, dressings, suture/staple removal.
-
Carrying out blood pressure checks, urinalysis and other appropriate clinical assessments.

Supporting contraception services (where appropriately trained).
-
Maintaining accurate, contemporaneous and confidential clinical records, including appropriate use of EMIS.
- Working collaboratively with other members of the multidisciplinary team.
- Working in accordance with NMC standards, practice policies, safeguarding requirements, infection prevention and health and safety procedures.

Contributing to the wider running of the nursing team and helping to maintain a high-quality, efficient and patient-focused service.
The
following are the core responsibilities of the practice nurse. There may be, on
occasion, a requirement to carry out other tasks. This will be dependent upon
Factors such as workload and staffing levels
a.
To
develop, implement and embed health promotion and well-being programmes
b.
To
implement and evaluate individual treatment plans for chronic disease patients
c.
To
identify, manage and support patients at risk of developing long term
conditions, preventing adverse effects to patients health
d.
To
provide routine nursing care to patients as required in accordance with
clinical based evidence
e.
To
provide wound care to patients including dressings and the removal of
sutures/clips
f.
To
implement vaccination programmes for adults and children and to administer
vaccinations (Baby / Child / Adult)
g.
To
provide travel medicine services where within scope of practice and competencies
h.
To
undertake Cervical Cytology Screening and support with recall of the same
i.
To
provide contraception services where within scope of practice and competencies
j.
To
take blood tests where required
k.
To
request pathology services as necessary and to process pathology results as
required and where within scope of practice and competencies
l.
To
undertake new patient and NHS Healthchecks as required
m.
To
provide chronic disease clinics, delivering patient care as necessary, referring
patients to secondary/specialist care as required where within scope of
practice and competencies.
n.
To
maintain accurate clinical records and ensure that snomed codes are used
effectively
o.
To
maintain chronic disease registers
p.
To
chaperone patients where necessary
q.
To
assist GPs with procedures when required
r.
To
prioritise health issues and intervene appropriately
s.
To
support the team in dealing with clinical emergencies
t.
To
support patients in the use of their prescribed medicines or over the counter
medicines (within own scope of practice)
u.
To
liaise with external services/agencies to ensure the patient is supported
appropriately (vulnerable patients etc.)
v.
To
delegate clinical responsibilities appropriately (ensuring safe practice and
the task is within the scope of practice of the individual)
w.
To
support the clinical team with all safeguarding matters in accordance with
local and national policies, reporting any concerns to the practice
safeguarding leads.
x.
To
deliver opportunistic health promotion where appropriate
Central Medical Centre is a well established forward thinking training practice located in Morden, Surrey.
The practice is situated close to Morden town centre, which has good public transport links including the London Underground Northern line and multiple bus routes.
The practice strives to provide high quality care to approximately 9700 patients in the local community and surrounding areas and is based over 3 floors.
We have an exciting opportunity for someone to join our multi disciplinary team to support our patients and clinicians. Our multi disciplinary team of 30 staff includes GPs, Practice Nurses, Clinical Pharmacists, HCA's, Mental Health Practitioner and Social Prescriber all of who are supported by an our non-clinical team that includes Patient Care Co-Ordinators, Receptionists and Administrators. The team work closely together to support each other in a friendly and welcoming atmosphere.
The practice is fully computerised and uses EMIS as it's clinical system.
Qualifications
Essential
- Registered Nurse
- Evidence of ongoing CPD
Desirable
- Post graduate diploma or degree (Primary Care)
- Qualified to undertake cervical cytology
- Qualified to administer baby/child immuisations
- Qualified to administer travel vaccinations
- Qualified to provide contraception services
- Qualified to manage Long Term Conditions
Experience
Essential
- Experience of working within a nursing team
Desirable
- Experience of working in General Practice or Primary Care environment
- Experience of working as a Practice Nurse or Community Nurse
