Practice Nurse

• Provide assessment, screening, treatment services and health education advice
• Maintain treatment room supplies and equipment
• Undertake collection of pathological specimens

• To carry out routine urine checks
• To carry out routine checks for height and weight
• To carry out routine blood pressure checks
• To carry out ECGs and report back to referring clinician
• To carry out routine cervical screening in line with national guidance and have responsibility for ensuring returned result (smear fail safe)
• To assess, plan and evaluate wound care interventions, and where needed seek guidance from senior colleagues when the individual feels they are working outside their scope of practice
• Assist in the management of post-surgical wound care, including but not limited to; Removal of wound closure products, clips and stitches
• Preparing and maintaining environments and equipment before, during and after patient care interventions, including assisting GPs during the performance of minor operations
• Assessing, planning and evaluation of chronic disease intervention, including; pharmacological and non-pharmacological on an annual basis in line with practice protocols and templates.
• Escalate chronic disease patients to secondary care colleagues and/or seek their professional advice to assist management of patients
• To offer signposting to lifestyle service, such as; smoking cessation, weight management, social prescribers
• To recognise deteriorating patients and refer to senior clinician for clinical advice and support
• Assist with hypertension reviews, staying up to date with national and practice guidelines
• Administer Childhood and Adult vaccinations under PGD (including travel vaccinations if qualified)
• Administer non vaccine injectable treatments, including but not limited too; B12, prostate cancer treatments, contraceptive injections, neuroleptic injections and specific medications controlled under secondary care PSD (e.g testosterone)
• Where appropriately trained; review annually Hormone Replacement Treatment (HRT) ensuring safety and adherence to national and practice protocol. Including the management of interim health questionnaires and prescription requests
• Oversee twice daily logging of fridge temperatures (manual check)
• To maintain adequate stock levels in their own clinical room
• In the absence of administrative colleagues, co-ordinate the ordering and rotation of clinical stock
• To undertake annual contraceptive reviews in line with FSRH guidelines
• Warfarin monitoring via INR star, including dosing and follow up. Ensure quality control checks, external and internal, are completed in a timely manner. Maintain accurate active patient list on INR Star.
• To support practice management with the maintenance of clinical equipment
• Take shared responsibility of administrative duties such as; docman tasks, health questionnaires and ad hoc nursing team tasks within EMIS, including any personal tasks
• Ensure accurate completion of all necessary documentation associated with patient health care including but not limited to; onward referral to either podiatry, audiology, leg ulcer clinic, lifestyle service
• In the absence of a Health Care Assistant, act as a chaperone when required.
• In the absence of a Health Care Assistant, assist in additional service clinics including but not limited to; minor surgery, family planning

• Current UK driving licence, access to vehicle and willing to undertake domiciliary visits.
