The Practice Nurse wilk work autonomously and as part of the wider team to deliver high quality care, including
- Management of long term conditions, including diabetes, asthma, COPD and Ischaemic Heart Disease
- Providing follow up and continuity of care for patients
- Delivering health promotion, preventative care, and self - management support
- Supporting the practice in achieving QOF targets and enhanced services
- Contributing to service development and quality improvement initiatives
- Support the management if long term conditions
- Provide appropriate health promotion for patients with or at risk of long term conditions
- Manage and treat long term conditions in line with most current national and local guidelines
- Cervical smear taking
- Routine childhood vaccinations
- Adult vaccinations
- Seasonal vaccinations (Covid/flu)
- Supporting the practice in achieving QOF targets and enhanced services
- Contribute to the provision of education and training for other staff and students
- Participate in research and audit where appropriate
- Maintaining accurate and contemporaneous records at all times
- Attend and participate in any meetings when required
- Any other delegated duties appropriate to the role
- Flexibility of working to ensure sufficient cover is provided for nurse holidays/sickness
The practice comprises of 2 GP Partners and 6 salaried GP's, 2 ANP's, 3 Practice Nurses, 1 HCA and 1 GPA. We also have an established highly skilled reception and admin team.
As a practice, we support the nursing team throughout their professional development as well as also being a training practice for Doctors in Training and Medical Students. The practice holds a strong belief in the importance of continuous professional development as well as personal wellbeing.
We work closely with our PCN colleagues including Pharmacist's, Pharmacy Technicians, First Contact Physio, Mental Health First Contact, CAMHS Mental Health Nurse.
Qualifications
Essential
- Registered Nurse (NMC)
- Significant primary care or relevant experience
- Management and education of patients with chronic disease/long term conditions
- Evidence of ongoing CPD and revalidation
- Certified in immunisations and vaccinations
- Cervical smear taking
Desirable
- Experience of working with EMIS clinical system
- Knowledge of QOF, NICE guidance and national screening programmes
- Excellent interpersonal and teamworking skills
- Excellent time management skills/ability to prioritise and manage own time
