As a Practice Nurse, you will deliver a wide range of treatment room and chronic disease management services, including cervical screening, wound care, venepuncture, ECGs, childhood and adult immunisations and vaccinations, spirometry, Feno testing, COPD, asthma reviews, hypertension management and health promotion activities.
You will support patients with the prevention and management of long-term conditions, undertake NHS Health Checks, and contribute to vaccination programmes and QOF achievement.
Working closely with GPs and the wider practice team, you will assess, treat and advise patients in line with practice protocols, maintain accurate clinical records, request pathology tests within your scope of practice, and support the smooth running of clinical services. You will also contribute to the management of clinical stock and equipment, infection prevention and control activities, and the effective use of our Anima Health digital triage system.

- Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
- Cytology Provide cervical screening and sexual health services as per contract requirements.

- Chaperoning and assisting patients where appropriate who are being examined by another clinician
- Requesting pathology tests, for example urine culture, swabs
- Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
Other tasks will include
- Maintaining and cleaning equipment used by the nurses and GPs
- Maintaining GP and Nurses rooms, stocking and rotating items as required
- Maintain general tidiness and cleanliness of nurses and treatment rooms
- Maintain stock of waiting room leaflets as instructed by partners and Practice Manager
- Ordering of vaccinations to maintain stock levels
- Participation in administrative systems in the Practice
- At all times there will be a need to maintain accurate records
- A duty to advise senior nurses of potential problems or errors within a range of assigned tasks
- Attend and participate in any Practice meetings when required.
Contribution To the implementations of Services
- To support the Infection Prevention Control (IPC) Team/Lead to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the Practice Manager
- To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
- As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
- Conduct chronic disease reviews (e.g., diabetes, asthma, COPD, hypertension) within your remit
- Ensure accurate coding and documentation to support QOF indicators
- Monitor and recall patients for annual reviews and follow-ups where required
- Support vaccination and immunisation programmes (e.g., flu, shingles)
- Support the team to improve QOF performance
- Support the Delivery of services under Enhanced Services (ES), Local Incentive Schemes (LIS), and Directed Enhanced Services (DES) within your remit
- Participate in NHS Health Checks and lifestyle interventions (e.g., smoking cessation, weight management etc).
- Contribute to data collection and reporting for contract compliance
- As required, advise potential diagnosis and manage acute and chronic conditions within your remit
- To review patient medications to enhance compliance in accordance to own scope of practice
- To request pathology services and process pathology results as required
- To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
- to maintain accurate clinical records in conjunction with current legislation
- To ensure read codes are used effectively
- To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
- To assist GPs with minor surgery when required
- To prioritise health issues and intervene appropriately
- To support the team in dealing with clinical emergencies
- To assist vaccination programme clinics for adults and children
The responsibilities described above are not exhaustive and may be amended from time to time in line with the needs of the practice. The post holder may be required to undertake other duties commensurate with the grade and scope of the role, subject to workload, staffing requirements and their level of competence.

- Experience of working in a primary care, community nursing, or comparable healthcare setting.
- Experience of delivering patient-centred nursing care.
- Experience of chronic disease management, including long-term conditions such as asthma, COPD, diabetes, or hypertension.
- Experience of undertaking wound care, including wound assessment and removal of sutures and staples.
- Experience of immunisation and vaccination programmes.
- Experience of ECG recording.
- Experience of health promotion and preventative healthcare.
- Experience of maintaining accurate clinical records using electronic patient record systems.
- Experience of working independently and managing a caseload within own scope of practice.

Qualifications
Essential
- Registered Nurse (Nursing and Midwifery Council)
- Flexibility to work outside of core office hours
- Disclosure Barring Service (DBS) check
- Occupational Health FIT clearance
- Meet the requirements and produce evidence for nurse revalidation
- Evidence of continuing professional development (CPD) commensurate with the role of a Practice Nurse
