We are seeking an enthusiastic and experienced Band 7 Physiotherapist to join our Hospital at Home (H@H) Service in Swindon Intermediate Care Centre. This is an exciting opportunity to play a key role in the assessment, treatment, and rehabilitation of adults with complex physical and functional needs, supporting recovery and promoting independence. Working as part of a multidisciplinary team, you will provide expert clinical leadership, deliver high-quality patient-centred care, and contribute to service development within a dynamic intermediate care setting. We are looking for a motivated clinician with excellent assessment and decision-making skills who is passionate about achieving positive outcomes and supporting patients to return home safely and independently wherever possible. Qualifications

Essential

  • Registered with the HCPC. Educated to degree level. Recognised teaching or mentorship qualification. Independent prescribing qualification. Patient assessment qualification (such as PACR). Significant previous experience in elderly care, frailty, community services or relevant acute care speciality. Clinical and technical skills in managing a range of clinical conditions. Proven skills in managing complex patient/service user caseloads. Experience of working as part of a MDT. Excellent written and verbal communication skills. Ability to confidently work autonomously and as part of a wider multidisciplinary team. Strong judgement and analytical skills, with the ability to interpret complex situations and make appropriate decisions. Ability to adapt to the demands of a constantly changing environment. Good IT skills and knowledge of clinical systems. Clean driving license. Car driver, access to a vehicle for daily use.
