Physical Health and Well-Being Nurse - Community Rehab

We have an exciting opportunity for a dynamic and passionate nurse to join our Community Mental Health Rehabilitation Team, using their holistic nursing skills to improve the physical health and wellbeing of adults accessing mental health services.

Working closely with patients, families and multidisciplinary colleagues, you will promote high standards of physical healthcare across community services. You will undertake holistic assessments, identify and manage physical health needs, develop and review care plans, and ensure care is safe, effective and evidence based.

You will support Mental Health Nurses and Healthcare Assistants to confidently deliver physical healthcare, providing guidance, supervision and training where needed.

The role also provides opportunities to contribute to audits, CQUINs, health and safety initiatives, service development and quality improvement projects. You will be encouraged to identify and implement improvements that positively impact patient care, outcomes and experience.

As this is a community-based role, regular travel across the county is required, including to locations that may not be accessible by public transport. Candidates will need to demonstrate how they can meet these travel requirements.
