Main duties and responsibilities (this list is not

serves a population of approximately 53,000 patients with a diverse

demographic. Our network is forward thinking and is actively developing our

network team of healthcare professionals. By working together, we use our

Our Network teams

Enhanced Care Visiting team for frail housebound patients

and those in care homes. The team includes visiting GPs, ANPs, Frailty Nurses,

Care Coordinators, Clinical Pharmacist.

BEC Treatment Centre working out of the Treatment Centre at

Shelley Manor Medical Centre supported by GPs, ANPs, Respiratory Nurse and

HCAs.

First Contact Physiotherapy Practitioner service.

Digital Lead

Pharmacy team comprising of Clinical Pharmacists and

Pharmacy Technicians.

Mental Health practitioners and Mental Health triage
