The role involves providing expert pharmaceutical care

through patient-centred medication reviews, medicines optimisation and safe

prescribing support. The successful candidate will manage patients with

long-term conditions, address medication-related concerns, promote self-care

and shared decision-making, and support repeat prescribing. As an NMP-qualified

professional, they will work collaboratively with multidisciplinary teams to

improve patient safety, health outcomes and the quality of services.

The successful candidate will provide expert pharmaceutical care, supporting patients across a range of clinical settings. Main duties of the role will include:

  • Conducting patient-centred structured medication reviews to optimise medicines use and improve health outcomes.
  • Providing higher-level clinical administration and medicines management support to ensure safe and effective prescribing practices.
  • Managing and supporting the ongoing care of patients with long-term conditions, using your independent prescribing qualification where appropriate.
  • Identifying and addressing medicines-related issues, improving patient safety and reducing unnecessary hospital admissions.
  • Working collaboratively as part of a multidisciplinary team (MDT), contributing your specialist knowledge to support holistic patient care.
  • Supporting the delivery of national and local clinical priorities, including prescribing quality initiatives and service improvement projects.

Qualifications

Essential

  • Completion of an undergraduate degree in pharmacy
  • Up to date registration with the General Pharmaceutical Council
  • Demonstrates and understanding of, and conforms to, relevant standards of practice
  • Follows professional and organisational policies/procedures relating to performance management
  • Hold an independent prescribing qualification

Desirable

  • Clinical Diploma

Experience

Essential

  • Experience and an awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practice
  • Experience of partnership/collaborative working and of building relationships across a variety of organisations
  • Experience of working in General Practice and / or PCNs

  • Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions
