The Paramedic will act as a specialist clinical
resource, working autonomously and flexibly within the PCN practices to assess, diagnose,
treat, refer, or signpost patients presenting with undifferentiated or
undiagnosed conditions related to minor illness or injury. Collaborating
closely with the wider multidisciplinary team, the role involves delivering
holistic, patient-centred care, supporting self-care and informed
decision-making.
The post holder will also be required to carry out home visits
as and when required and must have access to a vehicle.
Clinical
Provide
assessment, diagnosis and treatment at first point of contact by attending to
patients according to their needs. Majority of patient contact will be in clinics at practice, home visits will be as and when required
Act as an autonomous clinician, with the support and guidance from the
GPs
Proactively
identify, diagnose and manage treatment plans for patients at risk of
developing a long-term condition (as appropriate)
Diagnose and
manage both acute and chronic conditions, integrating both drug and non-drug
based treatment methods into a management plan
Accurately
triage and prioritise patients, including in emergency situations,
demonstrating the use of a variety of techniques to elicit the history of an
event/illness, including past medical and drug history
Recognise,
assess and refer patients presenting with mental health needs
Evaluate
clinical information from examination and history taking and initiate
appropriate treatment and / or referral, including admission to hospital if
required
Signpost
appropriately to other services in the identified healthcare pathway and to
refer patients to appropriate care settings, within locally agreed healthcare
pathways
Maintain
accurate and systematic documentation of patient interactions and clinical care
provided, including adverse events
Advice and
support members of the public on healthy lifestyles, health promotion and
disease prevention
Professional
Input to and extract information from clinical IT system
Work collaboratively with other members of the health care team
such as the, staff in Care Homes, Ambulance Service, and Secondary Care etc
Ensure that the wellbeing of patients and staff is maintained in
accordance with Practice and PCN policies. To undertake clinical audits in line
with the organisations clinical audit plan, and to participate as required in
equipment testing and other research
Contribute to the analysis of information to inform future
development of the service
Participate and contribute to clinical meetings
Identify risks to service provision and highlight them to the
Clinical Lead
Work within the requirements of Data Protection legislation as it
applies to patient records
Adhere to HCPC Code of Professional Conduct and at all times
practice within the scope of professional practice
Maintain current professional registration and indemnity

Qualifications
Essential
- HCPC Registration: Must be a registered Paramedic with the Health and Care Professions Council (HCPC).
- Recognised Paramedic Qualification:
- BSc (Hons) in Paramedic Science, or
- DipHE in Paramedic Practice (for those qualified before degree-level entry became standard).
- Proven ability to work flexibly to suit the needs of the PCN.
Experience

- Evidence of working in a community/primary care setting
Knowledge
Essential
- Understanding of health and social care processes
- Understanding of social determinants of health and how these can be addressed with patients
- High levels of health literacy
- Awareness of local services and resources for individuals, carers and families
Desirable
- Knowledge of a range of technology and digital tools that can be used support health and wellbeing
- Sound understanding of disease prevention (primary, secondary and tertiary) and how patients can build prevention into their lifestyle
- Excellent knowledge of Microsoft Office and Emis
- Understanding of current issues facing the NHS

- Able to provide clear and useful guidance and reports on complex matters to stakeholders at all levels
- Evidence of excellent clinical record keeping
- Ability to support patients during acute episodes of illness and to self-manage long term conditions where feasible.
