This job description is an outline of the tasks, responsibilities and outcomes required of the role. The job holder will carry out any other duties as may reasonably be required by the PCN Clinical Director. The following areas of responsibility are an indication of the role but could be subject to change as the impact of primary care network model evolves locally and nationally.The Manager will have specific duties coveringStrategy and Leadership -To oversee scoping, initiation, delivery, implementation, administration and evaluation of transformation projects on behalf of PCN as required:
Organisational Activity - to support the Clinical Director in ensuring the smooth running of the Network day to day.
Finance and Administrative - be the responsible lead for overseeing the Networks financial processing and budgets.
HR/Workforce Management - monitoring workforce requirements and supporting recruitment activity and planning.
QualityCompliance and Governance - Ensuring PCN's compliance with legal requirements for contracts, information governance, clinical governance and corporate governance.
Policy Development - to guide development of policies across the Network to promote efficiency. Facilitate the development of the neighbourhood agenda.
STRATEGY AND LEADERSHIP
- To oversee scoping, initiation, delivery, implementation, administration and evaluation of transformation projects on behalf of PCN as required
- Identify solutions to continue enhancing patient care within the evolving Neighbourhood. Advise the Network on effective digital solutions to aid transformation of services and workflow etc.
- Develop suitable digital solutions to identify and monitor population health needs.
- Set deadlines, assign responsibilities and monitor progress of project.
- Demonstrate value for money/efficiencies across the PCN related to the project
- Identify risks or issues and problem solve to find alternative solutions
- Implement and communicate changes across the PCN
- Chairing meetings such as Board or MDT.
- Review effectiveness and impact of projects
- Line Manager for the PCN employees
- Ensure proactive stakeholder management occurs including frontline staff, team, service, organisation, senior leaders, external bodies, attending meetings as a representative of the PCN.
- Provide and build upon key performance management data, as appropriate
ORGANISATIONAL
- Support the PCN Clinical Director to ensure the smooth running of the Network
- Submit and oversee mandatory returns
- Ensure the PCN has required policies and procedures and keep them functional and up to date.
- Work with the PCN Clinical Director for strategic and operational planning processes to deliver the PCNs contractual objectives
- Support the PCN Clinical Director with the writing of business and development plans as required
- Attend Thursday business meetings, highlighting current successes and challenges within the PCN.
- Present PCN digital and financial progress at monthly board meetings.
FINANCE AND ADMINISTRATION
- To be responsible for the oversight of the Networks financial systems/payments/forecasting and keep the PCN within its budgets
- To oversee financial transactions between the PCN and member practices.
- Oversee and check the accuracy and timeliness of any submissions for invoicing, Enhanced Services, payroll, or contractual requirements.
- Oversee accurate payroll, achieved on time, by third party provider.
HR/WORKFORCE
- To monitor workforce requirements for the PCN, looking for transformative solutions; regularly review budget and expenditure
- Support recruitment, induction and training of new PCN staff where required
- Embed and support PCN additional roles, supporting their role in transformation of patient care.
- Conduct staff touchpoint meetings, 1 to 1s, appraisals and team catch ups
QUALITY AND GOVERNANCE
- Ensure the PCNs compliance with GDPR and FOI along with other relevant legislation and professional information management standards such as GMC, BMA and LMC guidance
- To use digital solutions to monitor and report performance and quality within the Network
- Be the Information Governance Lead for the PCN
- Ensure that the PCN board and Clinical Director adhere to the corporate governance structure and requirements as set out in the Network Agreement and by Companies House.
- Alert PCN members to issues of quality and risk
POLICY DEVELOPMENT
- Facilitate development of neighbourhood agenda and services in preparation for expected Neighbourhood implementation.
- Review existing policies to identify similarities and differences between the Practices creating effective processes and policies across the network.
- Ensure all policies conform to relevant legislation and/or statutory requirements
- Support practices in engaging all relevant staff to embed the policies into practice

We are looking for a proactive transformation and operational manager to join our friendly PCN management team. The ideal candidate will
have the ability to work and prioritise tasks independently and contribute to
the wider PCN agenda and strategy, including Neighbourhood planning. You
will already have the skills, knowledge and work experience to meet the
requirements of this role and be able to communicate effectively with
colleagues and external stakeholders.
This is a permanent role for the Ascent Primary Care
Network, a group of three GP surgeries working together for their
communities across Milton Keynes and a small part of Bedfordshire. The PCN
consists of Asplands Medical Centre (Woburn Sands and Woburn), Walnut Tree
Health Centre and Fishermead Medical Centre. Although the role of Manager will primarily operate from the Asplands Medical Centre offices in Woburn
Sands, there is requirement to travel between the Network sites as well as home working on occasion. The role will offer comprehensive leadership workflow and administration to support to the PCN Clinical Director and Board, as well as supporting the functions of the wider
clinical team (ARRS team) across the PCN. The PCN and Asplands site team are welcoming and are based in a newly refurbished building with parking and easy access to local
routeways and village facilities.
