The Patient Safety Incident Response Strategic Lead is a varied and high-profile role. The post holder will lead the Trust's response to the most seri...

  • Lead on our PSII investigations and thematic reviews
  • Provide expert advice and guidance to staff leading other learning responses
  • Provide training to staff within the organisation of PSIRF principles / systems thinking to assist with the embedding of other learning responses
  • Analyse incident and investigation themes to identify trends and learning opportunities, contributing to Trust-level reporting (monthly/quarterly/annual) and informing prioritisation of improvement work.
  • Be one of the formal named lead Patient Safety Specialists in the organisation leading on or directly supporting patient safety improvement activity, ensuring that systems thinking, human factors understanding and just culture principles are embedded in all patient safety processes across HDFT.

Qualifications

Essential

  • Educated to Masters level or demonstrable equivalent experience in a relevant role.
  • post graduate patient safety qualification or equivalent

Desirable

  • leadership qualification
  • professional registration

Experience

  • worked in patient safety related role
  • knowledge and demonstrable experience of systems thinking / human factors/ just culture
  • evidence of leading investigation and writing reports

Desirable

  • trained PSS

Knowledge

Essential

  • compassionate engagement with pts and families after safety incident
  • understand system based investigation approaches

Desirable

  • datix experience
  • CQc standards and quality governance
