Job summary We are looking for a proactive and organised Patient Flow Facilitator to support safe, timely and person-centred transfers of care across our Community and Virtual Ward services. This is a key role within Compton Care, working closely with patients, families, acute hospital wards, community teams, social care and other system partners to help people receive the right care, in the right place, without unnecessary delay.
