- The post holder will provide clinical care within a team consisting of general practitioners, advanced healthcare professionals (ANPs, clinical pharmacist), practice nurses and Healthcare assistants.
- Patient contact will be mainly via triage, on-call duties and home visits
- Duties include, but not limited to daily duty triage, home visits, post-discharge reviews and minor illness management
- Provide care for presenting patient from initial history taking clinical assessment, diagnosis, treatment, and evaluation of care.
- You must demonstrate safe, clinical decision-making and expert care, including assessment and diagnosis skills for patients within the general practice.

Qualifications
Essential
- Qualified paramedic or specialist paramedic - FDSc or BSc qualified and registered with HPCP or Post registration certificate (Level 3) in first contact care
- Chronic disease management and mental health experience
- Completion of the Stage 1 and stage 2 of Health Education England (HEE) roadmap for Practice map - https://www.hee.nhs.uk/our-work/allied-health-professions/enable-workforce/roadmaps-practice-0
Desirable
- Accredited training in chronic disease management
- Non-Medical Prescribing qualification to undertake the professional role of independent/supplementary non-medical prescribing
- Mentoring/teaching qualification MSC level.

- Ability to analyse and interpret complex/ often incomplete information, pre-empt and evaluate issues, and recommend and appropriate course of action to address the issues
Desirable: - Independent thinker with good judgement, problem-solving and analytical skills; - Experience of clinical audits
Experience
Essential
- Minimum of two years experience of working in primary care or similar setting
- Experience in triage
- Experience of dealing with patients with undifferentiated and undiagnosed conditions
- Experience in triaging and managing minor illness and injury
- Evidence of delivering a service which improves quality and outcomes for patients
- Up to date knowledge and use of NICE guidelines for diabetes and other relevant national and local guidelines
- Experience in audit and research, 5+ years experience of providing leadership to a healthcare service working knowledge of the NHS, principles and values
- Recognises priorities when problem solving, identifies deviations from the normal pattern, is able to refer to seniors or GPs
- Work effectively independently and as a team player.
