Job description

Are you an experienced Occupational Therapist looking for your next locum opportunity within a fast-paced community setting? We are currently recruiting for a Locum Occupational Therapist to join a Discharge to Assess (D2A) service in Cheshire. This is a rewarding opportunity to support patients transitioning from hospital to home, helping to facilitate safe and timely discharges while promoting independence. Key Responsibilities:

Complete functional assessments in patients’ homes following hospital dischargeSupport safe discharge pathways and reduce hospital length of stayRecommend and arrange equipment and minor adaptationsWork closely with MDT colleagues including Physios, Social Workers, and community teamsDevelop patient-centred care plans to maximise independence Requirements:

HCPC registered Occupational TherapistExperience within community, discharge to assess, or intermediate care settingsStrong understanding of discharge planning and community rehabilitationAbility to work independently and manage a varied caseloadFull UK driving licence and access to a vehicle (preferred) This is a great opportunity for an OT who enjoys working in the community and making a real impact on patient flow and outcomes.
