Job description

You will be part of a supportive multidisciplinary team, working collaboratively, drawing on shared expertise, and appropriately delegating or signposting to other professionals to ensure the best possible outcomes for patients.
You will deliver and facilitate high-quality rehabilitation to support patients in achieving their individual goals, maximising opportunities for independence and enabling them to live meaningful and fulfilling lives.
Working in partnership with patients and their families, you will undertake comprehensive assessment, planning, implementation and evaluation of treatment and care programmes, using person-centred and strengths-based approaches to promote involvement, choice and empowerment.
You will support recovery and optimise independence by working collaboratively as part of a multidisciplinary team, contributing specialist occupational therapy expertise to ensure holistic, coordinated care.
Through effective communication, goal-setting and shared decision-making, you will help patients progress safely towards discharge while promoting self-management, confidence and long-term wellbeing.
